Recent 11 pound weight gain refractory to diuresis, LE edema, and nocturnal dyspnea (sleeps sitting), her past medical history is significant for atrial fibrillation, kidney dysfunction, and diabetes mellitus. Echocardiogram three months ago revealed ejection fraction of 18 percent.
