You are addicted to crack cocaine. You smoke anywhere from twenty-five to fifty dollars worth per day. To help finance your habit, you have begun to pawn important possessions including jewelry, stereos, and other household items. For the past two days you have not been at work. Because of your spotty attendance your boss insisted you receive an assessment before you can return to work. This is your second appointment.
You are addicted to Valium and Xanax (tranquilizers) and to two different pain killers (Vicaden and Codeine). Your normal daily dosage is four Valium, five Xanax, and three to four Codeine pills. The doctor began to prescribe these drugs as a result of a back injury in a car accident two years ago. Now the doctor is no longer willing to prescribe the drugs in the increasing dosages you feel you need so you are secretly seeing three different doctors for prescriptions. Your family found you passed out in the front seat of your car and insisted that you seek an assessment for your addiction. No one in your family is aware of how much you use. This is your second appointment.
You are a young, urban professional who works in a bank. You make a significant amount of money and have recently begun to attend parties where people are using speedballs (heroin and cocaine mixed together). You don’t feel that this is a problem because you only use on weekends.
